Required Documents

The documents listed below must accompany the Commander’s Checklist when the Soldier’s Command and the RSP Command are completing the signature blocks.  The documents must also be included when a copy of the packet when sending to this office:

• Personal Qualification Record (PQR) or Enlisted Records Brief (ERB) or DA 2-1 (Must show GT Score)
• Current DA Photo
• Unit Commander's Candidate Checklist
• DA Form - 705 Army Physical Fitness Scorecard
• DA 3822R - Behavioral Health Evaluation (Completed by a qualified military or civilian doctor)
• DA Form 1059 for NCOES
• DA Form 1059 for Drill Sergeant School (if attending the Recertification Course)
• DD 369 (Police Records Check)
• Last three DA 2166-8 (NCO Evaluation Report).  If you cannot provide the last three NCOERs, please provide a reason.
