Required Documents

The documents listed below must accompany the Commander’s Checklist when your Command is signing off on your packet and when it is forwarded to this office:

• One of the following:

Personal Qualification Record, Enlisted Records Brief (ERB) or DA 2-1

• Current DA Photo
• Unit Commander's Candidate Checklist
• DA Form 705, Army Physical Fitness Scorecard
• Mental Health Evaluation (Completed by a military or civilian doctor)
• DA Form 1059 for NCOES
• DA Form 1059 for Drill Sergeant School for those attending the Recertification Course
• DD 369 (Police Records Check)
• Memorandum by the Soldier stating the SF 86 and Fingerprint card has been completed and sent to Ft. Knox
• Last 3 DA 2166-8 (NCO Evaluation Report).
